Fellowship Program

The SDOH & Place Fellowship Program was created to guide and support public health, geography, and health equity leaders in developing a social determinants of health (SDOH) place-based data visualization (ex. story map, asset map, interactive map or classic dashboard) centered in a human-centered design (HCD) framework. These fellows read through each module of the Community Toolkit to develop a data visualization to help inform their research and their communities, along with providing necessary insight on how to improve our Community Toolkit for our users.
